Frequently Asked Questions

Physical Therapy Sessions focus on treating a specific medical diagnosis, injury, illness or medical condition.  Physical therapy is considered medically necessary and signed off by a physician or referring provider within 21 days of initiating therapy.

Wellness Sessions focus on optimizing movement, strength, balance, mobility, and physical performance rather than treating a specific medical condition. They are ideal for individuals looking to stay active, improve physical function, address areas of weakness or movement differences, and proactively support their overall health and wellness.

Both Physical Therapy and Wellness sessions are provided by Elizabeth, a licensed physical therapist.

Many sports wellness clients are children and teens. To help ensure their comfort and safety a parent or legal guardian is required to remain in the clinic for the entire duration of every appointment for anyone under 18.

To create an appointment for a minor:

  1. Book the appointment with the child's name, DOB & email
  2. In the notes for the appointment write the parent/guardian's name and contact information
  3. Create a separate account in Jane for the parent (without an appointment)

It is necessary for both the minor and parent to have accounts in Jane for paperwork, I will link the accounts together.  This keeps their medical information separate while appropriately sending the clinic waivers & HIPPA forms to the parent.

Click the Book Now button in the top right corner of the website. You’ll be prompted to choose either a Physical Therapy Evaluation or Wellness Assessment through a platform called Jane. For your first visit, please select one of these initial appointments rather than a follow-up session.

Jane is a secure online platform for scheduling, communication, forms, and clinical documentation. After scheduling, you’ll receive an email with instructions to create your account and complete the necessary paperwork prior to your appointment.  The paperwork should take about 10 minutes to complete.

  • Medical History
  • Financial Policies
  • Office Policies & Consent to Treatment
  • HIPAA Privacy Notice, Patient Rights & Communication Authorization
  • Appointment Specific Pre-Questionnaire

Jane does not require downloading an app, simply sign in through your web browser to manage your account and complete all necessary paperwork online.

If you have a referral, please attach it to your intake forms or fax it to:844-608-1395.

I can often accommodate appointments within the same week. If the times available are inconvient, please email me at elizabeth@elevate-evans.com and I’ll work with you to find another option.

Georgia law allows patients to seek evaluation and treatment from a licensed physical therapist with a Doctorate degree in Physical Therapy (DPT) without first obtaining a referral from a physician or other authorized healthcare provider. This is known as Direct Access.  Elevate Evans Physical Therapy & Wellness, LLC is a direct access clinic.

With your signed permission, after performing a direct access evaluation, your results will be sent to your physician or other healthcare provider.  Their signature is required for you to continue care for longer than 21 days from your initial evaluation.

Direct access is beneficial because it saves time and lowers medical costs.

There are two exceptions for direct access.  You may NOT book a direct access appointment with Elevate Evans Physical Therapy & Wellness, LLC if you have:

  • A new neuromuscular or developmental diagnosis/ condition in the last 9 months
  • A chronic musculoskeletal condition and have already received physical therapy in the last 90 days for the same diagnosis

In these cases you must have a referral from your physician or other authorized healthcare provider BEFORE booking an appointment.  Please contact the clinic for clarification or if you have any additional questions.

If you have a referral, your provider may fax it directly to the clinic at 844-608-1395.
